Description
ARA-290 (also called cibinetide) is a synthetic 11-amino-acid peptide derived from erythropoietin (EPO) designed to reduce inflammation and repair damaged nerves without increasing red blood cell production.

Research Highlights
Mechanism of Action: It selectively binds to the innate repair receptor (IRR)—composed of the EPOR/βcR heterodimer—to trigger anti-inflammatory and anti-apoptotic signaling pathways.
No Hematopoietic Effects: Unlike full-length EPO, it does not increase hemoglobin, hematocrit, or red blood cell counts, avoiding cardiovascular or thickening risks of blood.
Small Fiber Neuropathy (SFN): Clinical studies show promise in reducing neuropathic pain and improving corneal nerve fiber density in patients with sarcoidosis-associated small fiber neuropathy and type 2 diabetes.
Preclinical models suggest it dampens activated spinal microglia and astrocytes, alleviating mechanical allodynia and supporting axon regrowth.
Regulatory Status: Despite positive Phase II proof-of-concept trials, it remains an investigational compound and lacks formal FDA approval.
